Some of the most dangerous heart conditions announce themselves only once damage is done. They are inherited, they are invisible, and a great many people carrying them have no idea. Here is how genetic inheritance shapes cardiovascular risk — and why finding out early changes outcomes.

How genetic inheritance works

Each cell in your body contains 23 pairs of chromosomes — one set from your mother, one from your father. These carry the DNA instructions determining how your body is built and how it functions.

Just as we inherit eye and hair colour, we inherit traits influencing health. When DNA instructions are written correctly, the resulting proteins work as intended. Variants in the genetic code can introduce errors, and while most have no meaningful effect, some do.

For conditions inherited in a dominant pattern — which includes most inherited cardiac conditions — a person with one affected copy has a 50:50 chance of passing it to each child. That is a coin flip per child, not a guarantee that half of any family will be affected.

The genetic basis of cardiovascular disease

Heart health is significantly influenced by genetics, and some variants create a real predisposition to cardiovascular disease. Because these traits produce no visible signs, they can go undetected — and for some conditions, the first indication of a problem is a cardiac event.

The British Heart Foundation estimates that over 600,000 people in the UK carry a faulty gene that raises their risk of coronary heart disease or an inherited heart condition, most of them unaware.

Familial hypercholesterolaemia: the clearest example

Familial hypercholesterolaemia (FH) affects around 1 in 250 people — making it one of the most common inherited conditions there is. It causes very high LDL cholesterol from birth, dramatically accelerating the artery narrowing that leads to heart attacks, often decades earlier than would otherwise occur.

Two things make FH worth singling out. First, the large majority of people who have it are undiagnosed. Second, it is highly treatable — identified early and managed with statins and lifestyle measures, someone with FH can expect a normal lifespan.

Warning signs that warrant a conversation with your GP include a total cholesterol above 7.5 mmol/L, a close relative who had a heart attack before 60, or fatty deposits around the eyes or in the tendons of the hands or heels.

A note on "50% lifestyle, 50% genetics"

This figure gets repeated often, and it is too tidy. The genetic contribution varies enormously by condition — close to determinative for some single-gene conditions, and a modest nudge for common coronary heart disease, where lifestyle dominates. The honest summary is that both matter, and for most people most of the time, lifestyle matters more.

That is encouraging rather than deflating: it means inherited risk is usually something you can act against.

Prevention where it counts

  • Know your family history — particularly heart attacks, strokes or sudden death before 60, and unexplained deaths under 40
  • Have your cholesterol and blood pressure checked
  • Do not smoke
  • Stay physically active
  • Attend your NHS Health Check if aged 40–74

Where an inherited cardiac condition is confirmed in a family, NHS specialist clinics offer cascade testing with genetic counselling, so relatives can be screened appropriately.
